How do I report diseases?

Reporting diseases is as easy as 1-2-3!

1. Refer to the West Virginia Reportable Disease Protocols and Investigation Forms found in the Reportable Disease Protocol Manual
2. Complete appropriate forms and a West Virginia Reportable Disease Card (Referred to as the "Yellow Card").
3. Send the completed card to your local health department
